How It Works:

New Client Information Received
Once a new client’s information is received, our team reaches out to the family/individual for a consultation to introduce our services and answer any initial questions.

Intake & Enrollment
After the consultation, families complete a brief intake so we can learn more about their needs and confirm eligibility for services.

Program Orientation
Families are invited to an orientation (either in person or virtual) to meet our team and learn what to expect.

Personalized Family Plan
We work alongside each family to create an Individualized Family Plan (IFP), connecting them with the programs and support that best meet their needs
